In the Social Service position at JAG Healthcare Shelby, you will provide social service help to residents to attain and/or maintain their physical, mental and psychosocial well-being.
JAG Healthcare Shelby is a small facility with only 45-beds giving it a homelike feel for our residents.
This quaint environment also provides our nurses the opportunity to spend meaningful time with their residents without rushing from one room to the next.

Social Service Qualifications &
Responsibilities:
Licensed Social Worker At least 1 year Nursing Home Experience in Social Services preferred.
Maintains communication with residents and families.
Have a basic knowledge of the aging process, including misconceptions, physical changes and psychosocial aspects.
Has an understanding of applicable state and federal regulations.
Assists residents with arrangements for financial and legal matters Provides or arranges needed counseling services and psychiatric medication management as indicated.
Has an understanding of the rights of residents in long-term care facilities.
Assures documentation is accurate and timely in MDS care plans.
